If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, we accept cash, personal checks, Debit Cards, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

But Dr. Paul Freeman and our team at Atlanta Dentistry by Design do not want financial concerns to keep you from getting the care you and your family need. We work with most PPO plans and we invite you to call our office to confirm your coverage or to discuss your insurance options. Our team will be happy to review your plan with you to help you get the most out of your benefits. Please remember that payment is due at the time of your visit, and if you do not have insurance, need to make a co-pay or need to cover out-of-pocket expenses, our office accepts cash, checks and most major credit cards.

 Another option you may want to consider is Lending Club. This is a third-party company that works with our office and may be able to finance part or all of your dental care with low or no down payments or interest.
